Please fill out the form below. If submitted by 8 am on a weekday, your new Deacon OneCard should be available same-day. You will be notified via email when the replacement card is ready. 

Report Deacon OneCards lost or stolen immediately to University Police at extension 5591 after business hours or if there is no answer at the Deacon OneCard Office at extension 1949 during business hours.

1. The cost to replace a Deacon OneCard initially is $25; subsequent replacement charges will increase by $10 each time, up to a maximum of $55.
2. These fees apply for replacement of cards that are lost, stolen, damaged, or malfunctioning, and other circumstances concerning your Deacon OneCards.
